There is one that should help:

The idea of installation is the same as for Office365, you just need to use your own server URLs for configuration.
Also if you are using Office365 account then if you'll configure the plugin there it will automatically appear in your Outlook. Basically configuration is the same for OWA and Desktop version of Outlook, it is performed via web interface.
