#3584 - Inbound SMS and calling is not working

I've installed RT Telephony and and it's working for outbound calling and SMS, but I'm getting the following errors for incoming calling and SMS. Any idea on what could be going wrong? I've sent my Twilio and Sugar account details to your support email. Thanks.
